Summary Job Description
Under the direction of the District Nurse, deliver health related care to students in the district, specifically grades PreK-2nd grade. The position is responsible for the coordination, monitoring, and oversight of health and nutrition services for preschool programming which will include monitoring, tracking and reporting health and nutrition data to meet local, state, and federal expectations and requirements.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Health Services
- Implement and monitor a health services tracking system for required vaccines, health checks, and screenings including completing follow-up procedures when needed.
- Monitor and maintain health records to ensure proper follow-up is completed.
- Provide timely submission of local, state, and federal reporting of health requirements to ensure compliance including but not limited to Head Start, Universal Preschool and other programming reports.
- Prepare health summaries for preschool families three times a year, including height and weight measurements.
- Provide appropriate communication and notifications to parents and stakeholders in all areas of health screening and referrals.
- Make child health plan referrals when appropriate; Communicate with parents/guardians of students on health care plans; Collect and maintain documentation of care provided and progress of interventions.
- Establish and maintain a Health Services Advisory Committee (HSAC) to support children’s healthy development. The HSAC is an advisory group usually composed of local health providers who represent a wide variety of local social services agencies who develop and review health policies and procedures. They may include pediatricians, nurses, nurse practitioners, dentists, nutritionists, and mental health providers. Head Start staff and parents also serve on the HSAC.
- Connect enrolled families with referrals and resources to community partners for health related concerns.
- Communicate any health screening result concerns promptly to the appropriate building teams as needed.
- Participate in MTSS, 504 and IEP meetings as appropriate.
- Effectively manage medical needs with students to maximize instructional time.
- Communicate with families regarding community health events in Lake County.
- Negotiate and renew collaborative agreements with local health providers.
- Conduct or arrange for health education for families and staff. Ensure health education is incorporated into classroom activities.
- Organize and host two preschool family fun nights (one per semester) with a health focus in collaboration with Elementary staff.
- Attend and manage care for students while off campus on field trips.
Nutrition Services
- Coordinate with the preschool education manager/lead to provide preschool teaching staff with materials and training to support nutrition programming in the classroom.
- Collaborate with the school district food service manager and nutritionist regarding food substitutions or food allergies of enrolled children at the preschool level.
- Collaborate with the Preschool Director for all CACFP guidelines and functions as it relates to the classroom.
- In collaboration with the preschool Director, provide support to kitchen staff connected to CACFP guidelines and family style meals in the classrooms.
Program & Administrative
- Maintain first aid kits, health supplies, and red emergency bags for the Preschool program.
- Participate in preschool management team meetings to provide input, participate in on-going monitoring of program, support problem solving, and leadership of the program.
- Provide data for reports
- Participate in professional development that is job related.
- Promote a positive school climate and follow school norms and expectations.
- Maintain confidentiality and professional boundaries.
- Perform other duties as assigned.
Qualifications:
● Preferred experience with Health related care
● Preferred experience in K-12 educational setting
● Work with students from diverse backgrounds and a variety of family situations
● Commitment to following school-wide expectations
● Knowledge of classroom discipline
● Bilingual English/Spanish preferred
